Here at Daleville Chiropractic, we do our best to make you feel at home and help you receive the highest quality care possible. When you walk through our front door, you will be greeted with a smile by our friendly office staff. Click here and fill out our patient intake form on your smart phone, tablet, or computer. This will not only save you time on your first visit, but also give the doctor the opportunity to get to know more about you and your condition prior to your appointment. Based on the information you provide on the intake forms, there may be additional, but minimal forms for you to electronically fill out in our office.
Once we have gathered all necessary information from you, you will be accompanied to the exam room by one of the office staff. There our doctor will talk with you about your medical history and then you will receive a thorough new patient examination. The examination involves simple physical tests and measuring your range of motion. From that point, the doctor will decide whether or not it will be necessary for you to receive spinal X-Rays. For your convenience, we have an X-ray unit on site and your X-rays will be taken immediately following your examination. These X-rays are necessary for the doctor to get to the root cause of your problem without making any guesses as to what your spine looks like, providing them with the knowledge of how to adjust your spine the best way possible. To conclude your new patient visit, you will be accompanied back out to the front desk where our staff will schedule you for a Report of Findings.
